About Patient Portal

The Follow My Health secure patient portal is a universal health record. This means that you can create one account in Follow My Health and use it to access your records for any office or facility that participates with Follow My Health. With this account, you can access your health records wherever there is an internet connection. A free downloadable app is also available for use on your smart phone or tablet.

Use the portal to send your physician a message, request appointments, view lab results, and receive appointment reminders.

How do I sign up for the patient portal?

Call our office to request an email invitation to join the patient portal.

You will receive an email from Follow My Health, inviting you to create an account.

Click on the link and follow the steps.

How do I choose a login method?

You are given several choices of log in method when you create an account. We recommend using the green Follow My Health shield.  Whichever method you choose, this is the method you will use EVERY TIME you log in.

What is my Invitation Code?

When you are creating your account, you will be asked for your invitation code.  This is your 4 digit birth year or the last 4 of your SSN.

How do I access my blood work results?

Log into your account. Click on My Health, then Results.

Why can’t I see any of my information?

Please make sure your account is connected to Prism Medical Group.

  • Click on My Account at the very top of the screen. Click on the top choice in the drop down, My Connections.
  • The next screen will display the facilities you are connected to.
    • If your screen says, Prism Medical Group – Pending, please click on the x at the right to remove this request and request a connection again (see below).
  • If you are not connected to any organizations, click on Add organization. Search for Prism in the list and follow the prompts to connect your portal account to your health record.

Why can’t I see my most recent lab results?

Lab results will not show up in your portal account until they have been reviewed by the doctor. This can take a couple of days.

If it has been more than a couple of days, you have received an email that your results should be there, but they aren’t there, please check to be sure your account is connected to Prism Medical Group (instructions under “Why can’t I see any of my information?” If you still have questions, please call our office for assistance.

How do I send my doctor a message?

Click the black link at the top your home page that says, “Send a message”.

Can I set up my Follow My Health account with a mobile device?

Yes, you can.  Click on the link in your email invitation and follow the prompts to create your account.  When you are done, you will be prompted to download the free FMH Mobile app.

What if I forget my username?

If you have forgotten your username, please call our office.  We can send you an email with your username, and a link to change your password, if necessary.
