The Follow My Health secure patient portal is a universal health record. This means that you can create one account in Follow My Health and use it to access your records for any office or facility that participates with Follow My Health. With this account, you can access your health records wherever there is an internet connection.

To use Follow My Health on your smart phone or tablet, please be sure to download the free FMH Mobile app from the App store or Google Play.

Use the portal to send your physician a message, request appointments, view lab results, and receive appointment reminders.

The portal email should be used for informational purposes and non-urgent health updates. Please be aware that providers are in the office evaluating patients during business hours and portal messages are not monitored on an urgent basis. Depending on the complexity of your request, an e-visit (E-mail message chain) may be billed to your insurance carrier. If the insurance does not cover this service then you would be responsible for the charges, which could range from $45-$160. Time spent reviewing records and obtaining information, providing recommendations, prescribing medications and ordering tests will all be considered when deciding on a billing level.

1How do I sign up for the patient portal?

Email: support@prismmedicalgroup.com to request an email invitation to join the patient portal.

You will receive an email from Follow My Health, inviting you to create an account.

Click on the link and follow the steps to create your account.

2How do I choose a login method?

You are given several choices of log in method when you create an account. We recommend using the green Follow My Health shield. Whichever method you choose when creating your account will be the same EVERY TIME you log in.

3What is my Invitation Code?

When you are creating your account, you will be asked for your invitation code. This code will be in the invitation email that you receive, and is unique to each individual.

4How do I access my blood work results?

Log into your account. Click on My Health, then Results.

5Why can’t I see any of my information?

Your account is not connected to Prism Medical Group. This will happen if your account is created without you receiving an invitation from our office. Please email support@prismmedicalgroup.com to request an invitation and complete the connection process.

6Why can’t I see my most recent lab results?

Lab results will not show up in your portal account until they have been reviewed by the doctor. This can take a couple of days.

If it has been more than a couple of days and you have received an email that your results are available, but you can’t see those results, please call email support@prismmedicalgroup.com for assistance.

7How do I send my doctor a message?

Click the black link at the top your home page that says, “Send a message”.

8Can I set up my Follow My Health account with a mobile device?

Yes, you can. Click on the link in your email invitation and follow the prompts to create your account. When you are done, you will be prompted to download the free FMH Mobile app.

9What if I forget my username?

If you have forgotten your username, please click on the “Forgot username” link in Follow My Health. You will receive an email with your username.

10What if I forget my password?

If you have forgotten your password, please click on the “Forgot password” link in Follow My Health. You will receive an email with a code to use to reset your password.

11Why do I receive an error when logging in through the app?

If you receive an error when logging in through the app, even after resetting your password, please make sure your app is updated. Go to the app store or google play, and update the app to the most recent version.

12Do I need Follow My Health if I have Beaumont MyChart?

You can use Follow My Health to send a message to your provider, request a medication refill or referral, ask billing questions, and view your test results. Some of your test results will be available in Beaumont MyChart, but not all. It is best to have both accounts.
